Our site contains forms for online registrations, symptom questionnaires, feedback and contact requests.

We collect identity and contact data when supplied to us through people making enquiries and requesting to be contacted from a member of Intus staff or for information. We collate this information to enable us to respond to enquirers and send out information and advice as requested.
