Our site contains forms for online registrations, symptom questionnaires, feedback and contact requests.

The information supplied to us through the Patient Registration form and Bowel Symptom Questionnaire gets sent directly through to our medical records and is not stored anywhere else. The information is being received, collected and held securely by Intus Ltd, PO Box 36692, Merivale, Christchurch.

These forms collect personal information about you to assist Intus with your admission, treatment and care.

We collect identity and contact data when supplied to us through people making enquiries and requesting to be contacted from a member of Intus staff or for information. We collate this information to enable us to respond to enquirers and send out information and advice as requested.

No information is shared or sold to any third-party company. We will immediately remove you from our database if requested.
